Abstract:

Low back pain is a significant cause of morbidity and disability. It is one of the most frequent reasons for clinic visits, loss of work hours and billions of dollars of expenditure in treatment and wage compensation. Although low back pain is complex in aetiology, chronic low back pain is commonly associated with degeneration of the intervertebral disc (IVD). Patients with severe IVD degeneration and pain often require surgery with its associated morbidity.Adult IVD cells have similar biological phenotype to chondrocytes1, using this knowledge various biological therapies for IVD regeneration have emerged. This includes injection of biomolecules such as OP-1/BMP-2/GDF-5 2-4 and adenoviral delivery of Sox9 gene 5 into degenerated discs. Chemical genetics is an emerging technique that uses small chemical compounds to alter the functions of proteins to which they bind. The principle is similar to how genes are manipulated in knock-out or transgenic animal models to understand the function of a particular gene; the downstream effects of alterations of these proteins can be evaluated to understand the function of the protein and its pathway, hence the term "Chemical Genetics". It is thus used to identify which proteins regulate different biological processes, to understand in molecular detail how proteins perform their biological functions, and to identify small compounds that may be of medical value. Through strategic design of screening assays, this technology has been successfully employed to identify potential pharmaceuticals for cancers 6, 7, diabetes 8, cardiovascular diseases 9, and neurodegeneration 10, 11. The Department of Microbiology at The University of Hong Kong has the only high throughput chemical genetics screening platform in Asia, and they have successfully used this technology to identify compounds that have anti-SARS-coV and anti-angiogenesis activity 12, 13.Using the principle of "Forward Genetics", we aim to perform a primary screen of a library of over 50,000 small compounds using goat chondrocytes, and to select those that are able to maintain or increase proteoglycan levels. Recent evidence that a number of molecular pathways are involved in IVD degeneration, and that an orally administered drug, Esculetin can reverse IVD degeneration in rabbits 14, would support that other small compounds and pathways that may act similarly or even more specifically can be identified. Abstract:

Degeneration of the intervertebral disc (IVD) is attributed to senescence and/or apoptosis of cells and downregulation of synthetic activities. Little is known about how these processes are regulated in IVD cells. MicroRNA (miRNA) genes have been recently recognized to play important roles in various physiological and pathological processes by their powerful regulation of gene expression, and is also known to be expressed in cartilage. Whether they are involved in the regulation of IVD cells is not known, but this information may provide new insight into the regulation of IVD degeneration and therefore ways to halt or regenerate it. This proposal aims to develop this new technology and assess the presence and regulatory roles of miRNA genes in IVD cells in comparison with chondrocytic progenitors cells by: 1) To assess and compare the expression profile of miRNA genes in nucleus pulposus (NP), annulus fibrosus (AF) cells and cartilage of young, adult and aged rats. 2) To assess and compare the expression profile of miRNA genes in NP and AF cells of human subjects.

Abstract:

Objectives :To establish an aminal model where a vascularised nerve graft can be used to bridge a complete transection of the spinal cord.Key issues and problem being addressed :In the past 20 years or so, attempts to achieve regeneration of the spinal cord across a defect left after spinal cord injury were directed towards three aspects :-a. To promote regeneration of the nerves by various active substances such as growth factorsb. To neutralise the inhibitors of regenerationc. To bridge the gap with all kinds of conduits and cells.The use of the peripheral nerve graft, which is highly successful in the peripheral nervous system, may be able to help bridge the gap, provide growth factors, or even neutralize the inhibitors. Indeed, Cajal, Kao, Wardrope, Cheng had all tried to use peripheral nerve grafts in different ways, yet only with rather feeble regeneration. One of the reasons for their failure, perhaps, was the lack of a good blood supply of the transplanted nerve graft in the environment of the spinal canal where the bony and ligamentous surrounding and the CSF were not conducive to re-vascularisation, thus leading to necrosis and fibrosis of the grafts. The use of vascularise nerve grafts (VNG) may therefore open up a new approach in spinal cord regeneration.The principal investigator had explored the use of VNG in spinal cord injury in the rat. A graft of the median and ulnar nerve of the upper limb was harvested with a long pedicle of blood vessl and was directed towards a defect of the spinal cord at the cervico-dorsal junction. Initially, a total transection of the cord was created, but all the animals died. Even with a hemi-transection model, 50% of the animal died unless very careful post-operative care was instituted. The subsequent study of the regeneration through the graft had to be carried out with the remaining intact half completely divided in order to show neurotransport and electrical conduction across the grafts. Extremely painstaking care and highly demanding technique was required. With such an amimal model, it was possible to show extraordinary good regeneration into the graft when compared to non-vascularised free nerve grafts (FNG).This animal model with a hemi-resection of the spinal cord, however, is open to all kinds of criticism. It will be ideal to develop an animal model where a VNG can be used to bridge a complete transection of the spinal cord.In trying to establish a VNG for a complete transection of the spinal cord, it is almost certain that the only feasible site for a complete cord transection in the animal has to be performed below the mid-thoracic level as a higher level will incur an extremely high mortality rate. In this area, vascularised nerve grafting post 2 major problems :-(1) Initial exploration showed that pedicled nerve graft from both the upper limb and the lower limb could not reach this area in the rat and rabbit.(2) If free vascularised grafts are used, the recipient blood vessels are too small for microvascular anatomosis even in the rat.

Abstract:

The objectives of this study are 1) to develop new methodology of myoelectric signal acquisition, decomposition and processing in sensor array sEMG for discovering and exploration of in-vivo functional physiology of muscle, 2) to characterise and compare the in-vivo motor unit firing behaviour and coordination processes corresponding to the actual pathophysiological status; and 3) to investigate the pain behaviour and electrophysiology of neuromuscular system in healthy, myopathic, spinal cord compression, and nerve root compression condition. Pain is the most common clinical symptom in various pathological conditions, which usually affects neuromuscular electrophysiological patterns. However, the pathomechanism of the effect of pain on neuromuscular activation potential distribution and their coordination behavior are still unclear. The key issus of this study is to find various myoelectrical activity patterns of the insulted muscle area, which will establish a reliable model to simulate the pain effcts on muscle activation and muscle contraction behaviour.

Abstract:

(1) Purpose of investigation: Intraoperative spinal cord monitoring can significantly minimize the risks of iatrogenic injury to spinal cord during spinal surgery, neurosurgery and aortic surgery. Somatosensory evoked potential (SEP) is the most popular technique currently being used in the operating room. The ensemble averaging method (EA) is the only practical method to extract SEP, but it will considerably lengthen the surgical time and hinder the surgical procedures, and more importantly, it cannot provide the spine surgeon with a timely warning of the eminent danger of cord injury. This motivates us to study and develop a new and improved method for fast extraction and automatic recognition of SEP.(2) Key issues and major problems: The main problem in SEP monitoring is its extremely low signal to noise ratio (SNR) and variability in its signal waveform. The EA method is used to extract SEP from background noise, but it is time-consuming. Moreover, the noisy waveform makes the auto-recognition of abnormal changes in SEP a difficult task. Sometimes, latency/amplitude measurement based on time-domain SEP waveform is highly variable.(3) Objectives and Possible outcomes: This project has 3 main objectives: i) To study and develop a signal processing technique for improving the SNR of SEP signals. We shall study and apply a multi-adaptive filter in combination with a novel data-reusing non-linear method based on radial basis function neural network (RBFNN) for improving the SNR of SEP signals. (4) ii) To study and develop a fast extraction method for separating the SEP signal from other interference based on multi-channel blind source separation. The use of a multi-adaptive filter can improve the SNR of SEP signals, but it may not be sufficient to obtain a reliable SEP signal by a single trial. The multi-channel blind source separation (BSS) will be used to enhance the performance of the algorithm so that a more reliable and reproducible SEP can be obtained from every single trial. Moreover, the time-frequency feature of SEP will be investigated to establish a time-frequency model for auto-classification and identification of SEP waveform. (5) iii) To investigate and develop an auto-recognition method based on time-frequency analysis of SEP. The developed fast extraction and auto-recognition methods will be applied to clinical and animal experiments and the specificity and sensitivity can be then evaluated. (6) Long-term impact, relevance, significance and value: The findings of this research will contribute to the further development of fast and reliable techniques for SEP monitoring. The ultimate goal is to provide safer and more reliable monitoring, which is more easy to carry out during surgery. In addition, the results of this research would benefit the further development in SEP diagnosis of neuromuscular disorders. On the engineering perspective, the signal processing algorithms and results so developed will have considerable impact on the theory and practice of multi-channel signal processing, especially in neural engineering and electroencephalograph extraction. Last but not least, the future application of this study can be developed into a product with economical benefit for Hong Kong.

Abstract:

To develop a new material based on water-absorbing polyurethane elastomer encased by sustained-release membrane, which can expand it volume under well control; to establish a new animal model of cervical spondylotic myelopathy with chronic and progressive compression to the spianl cord; to testify the feasibility and reliability of this new animal model by functional MRI, radiological and electrophysiological examination. Cervical spondylotic myelopathy (CSM) is a common spinal discorder caused by degeneration of the cervical spine, impairs motor and sensory functions insidiously and progressively. However, the exact pathophysiology and precise mechanisms of the disease process remains to be elucidated. The key issue and problem addressed here is the difficulty to reproduce the chronic spinal cord compress on animal model. This study is to establish a new animal modle with progressive compression on the cord in a controllable duration and designed profile.

Abstract:

The objective of this small project is to investigate the feasibility and the difficulties in the use of bio-corrodible alloys and their composites, particularly magnesium alloys and their composites, as a temporary implant material in the body when used in a significant amount, to facilitate the repairing of body tissues.Animal work will determine the "in vivo" compatibility of such biomaterials, and cell culture will suggest whether such alloys and composites can promote the growth of cells other than osteoblasts. Investigation on composites will suggest the research direction in the reduction or elimination of the gas evolved from the alloys and trapped under tissues.

Abstract:

1) Introduction: Low back pain has been a major focus in clinical practice. It represents a world-wide health problem of which about 70-80% population will experience low back pain in their lifetime [1,6]. Intervertebral disc degeneration has been suggested to associate with low back pain and sciatica and is thought to have multi-factorial causes. Previous study in human indicated that 90% of people will develop intervertebral disc degeneration above the age of 50. However, whether the degeneration is caused by natural aging or other unknown pathological conditions is still not clear [2]. Intervertebral discs (IVD) are avascular fibrocartilagenous structures between the vertebral bodies of the spine which function to articulate the spinal motion and are subject to high mechanical loading. Degeneration of IVD is generally characterized by changes in disc morphology, composition of extracellular matrix (ECM), loss of proteoglycan and water content, and an up-regulation of metalloproteinases in IVD [2,8-10]. Nonetheless, there is a lack of consensus on the cause of degeneration. In particular, whether the degeneration is a result from a loss of mechanical integrity in the disc or the mechanical loading itself being a precipitating factor of the degeneration is still controversial [6,7]. Studies have also suggested that the degeneration is related to a change of disc cells response to mechanical stimuli [11], but the molecular events happened during the response have not been clearly elaborated. IVD consists of three components: an outer fibrous ring (annulus fibrosus, AF), an inner gelatinous filling (nucleus pulposus, NP), and cartilaginous endplates (EP) which sandwich the former two structures. During organogenesis, NP is developed from the transformation of notochord and AF is differentiated from the condensation of sclerotome, implying that IVD is derived from mesodermal origin [3,4]. IVD is populated by three major cell types: chondrocytes in EP, notochordal/chondrocyte-like cells in NP and fibroblast-like cells in AF. NP and AF contain abundant ECM, particularly collagens and proteoglycans, which are present in different proportions in the two structures [6]. Cells in IVD have been defined base on their morphology, intercellular organization, and gene expression. NP is composed of a heterogeneous cell population of interconnected cells with prominent vacuoles, which are thought to be originated from the notochord, and small chondrocyte-like cells from yet unknown origin [12]. The notochordal cells appear to be replaced with chondrocyte-like cells during IVD maturation. The replacement of cells may be possibly due to apoptosis of notochordal cells together with an invasion of chondrocytes from AF or EP, or due to differentiation of notochordal cells into chondrocytic phenotype. Study has demonstrated that degeneration is not observed in IVD where notochordal cells persist to adult age, suggesting notochordal cells play vital role in maintaining IVD integrity [21]. Among various species studied, mature sheeps, dogs, and rats are suggested to retain NP cell populations mimicking those in adult humans [12]. In situ expression of variety of genes has been studies in NP. They express chondrocyte markers such as SOX9, collagen II and IX, agreccan, decorin, TGFbeta, and Ptc [13-18]. Gene knockout in mice has demonstrated that Sox5 and Sox6 regulate NP formation while Col2a1, Pax1/Pax9, Bapx1, and Jun play regulatory roles in the development of vertebral column [4,5,19,20]. To-date, the developmental regulation and specific function of NP in IVD are still largely unknown. In particular, there have been no genes found to be specifically expressed in the notochordal cells which can define their identity and distinguish them from chondrocyte-like cells in NP. 2) Key issues and problems: There has been limited advancement in the molecular biology of prenatal and postnatal IVD. Insufficient molecular characterization of IVD cells in is one of the major obstacles in understanding the properties of IVD and the etiology of disc degeneration. The lack of knowledge in NP cell-specific genes inevitably hampers the dissection of molecular pathways related to the IVD formation, maintenance, and the cause of IVD degeneration. On the other hand, we and other groups have been working on biological therapies such as the use of bioactive molecules and stem cells to regenerate disc cells in degenerated IVD. Without sufficient knowledge in the molecular signatures of the cells, accessing the outcome of the therapies will be difficult because cell phenotype in the treated IVD cannot be adequately described. In addition, the lack of cell-specific markers also compromises the use of high-throughput screening technologies that may rely on molecular cues to report expression changes specific to IVD. Taken together, the issue is critical to both basic and applied IVD research. 3) Hypothesis and objectives: It is hypothesized that notochordal cells in NP are specialized cells for IVD function and express unique genes which determine their specific differentiation status and cellular function. This study aims to identify the genes specifically expressed in notochordal cells in NP by the use of transcriptomics and bioinformatics.

Abstract:

(1) To develop a novel strontium-hydroxyapatite nanowhisker (Sr-HA-Nano) reinforced bioactive bone cement. (2) To provide a complete mechanical and biocompatible profile of novel Sr-HA nanowhisker cement follow ISO 5883 and ISO 10993 standards. The key issue of this study is to improve bending and fatigue strength of the current Sr-HA bioactive bone cement. Based on literature review, the weak filler fracture toughness, the microvoid formation during mixing, and the filler aggregations are the major cause of lower mechanical strength. One solution to solve these problems is to reinforce the cement with the stronitum hydroxyapatite nanowhisker and by surface treatment using acrylolpamidonate. To further increase the mechanical strength of the novel bioactive boen cement, we will apply surface treatment to the inorganic fillers using acrylolpamidonate which will increases filler surface hydrophobicity assisting dispersion within non-polar resin phase.

Abstract:

The purpose of this study is to examine and compare the pullout strengths of pedicle screws from bioactive strontium-containing hydroxyapatite bone cement (Sr-HA) and polymethylmethacrylate bone cement (PMMA) in different insertion timings. The pedicle screw is a component of spinal implants commonly used in spinal fusion for various diseases such as scoliosis, infection and tumour. In patients suffering from osteoporosis or a tumour, the bone quality of their vertebrae would be weakened and render the fixation of a pedicle screw unsecured. A surgeon might utilise bone cement, classically PMMA, to augment the screw purchase. PMMA is not a glue, rather, is an inert void filler. It cannot encourage bone ingrowth and might even fail with time due to mechanical fatigue. On the contrary, the bioactive Strontium-Hydroxapatite bone cement, which was developed, patented and commercialised by the Department of Orthopaedics and Traumatology at Hong Kong University, might surpass the traditional PMMA by the virtue of its unique ability to promote bone ingrowth. Even for the classical PMMA bone cement, there have been only a few articles concering the pullout strength of screws. Flahiff et al discovered that the pullout strength was highest for screws inserted into doughy cement and lowest for screws inserted into hard cement. Apart from the strength, the modes of failure were also different. It was postulated that the timing of screw insertion cast an effect on interdigitation and hence the strength in screw-cement and bone-cement interfaces. However, as the experiment carried a lot of variables, conclusion of the best timing could hardly be arrived at. To simplify the model, Motzkin et al utilised cement blocks for mechanical test. They found a significantly stronger pullout strength for screws placed in fluid and solid polymethylmethacrylate (PMMA). Critics arose as they only tested cement blocks with a thickness of 8mm, which was not strong enough to resist screws pullout without breakage. Heller et al refined the procedure and used a sizable cement block to test the pullout strength. They documented the strength was much higher than recorded from Motzkin study, sometimes it was the screw broke rather than the cement block gave way. Nonetheless, they did not detect the change of pullout strength for screws inserted at different timings. Currently, there is no concensus on the best timing for screw insertion. Thus there is obviously an urge for a study with refined methodolgy and systematic reporting. From pilot studies, for screws inserted 10 mm into bone cement, the Sr-HA cement has shown to have an average pullout strength of 6500 N when the screw is inserted 4 minutes from mixing, better than that of PMMA, which is 5400 N. At 8 minutes though, while the PMMA's strength increases to 5700 N, the Sr-HA's average pullout strength decreases significantly to 2600 N, most likely due to the Sr-HA's fast setting properties. However, we hypothesize that the pullout strength is actually higher when taking into account the ability of the Sr-HA to promote bony ingrowth.

Abstract:

(1) Purpose: This study aims to apply state-of-the-art techniques to study the micro and nano mechanical properties of both normal human intervertebral discs and discs at different grades of degeneration, producing a database available for future reference. (2) Objectives: (a) Testing the mechanics of a bundle of collagen fibrils from Annulus Fibrosus of human intervertebral discs; and (b) Mechanically testing the individual collagen fibrils at nano-level by stretching the fibrils in force spectroscopy mode (contact mode) of the Atomic Force Microscope (AFM). (3) Key issues: Low back pain (LBP) is a common and significant problem worldwide. Compensation for occupation related injuries and LBP in Hong Kong alone amounts to about HK$675 million a year. A recent survey suggested Hong Kong is one of the most LBP-afflicted communities in the world, with a cumulative life prevalence of 57% for all age groups, and peaks at ages 45 to 54 with a life prevalence of 68%13. (4) Degenerative disc disease (DDD) is one of the most common causes of LBP and sciatica. Current methods of treatment do not address the underlying problem of degenerated and non-functional discs, but rather by-pass it by spinal fusion. Recently, there has been substantial interest in developing artificial discs by tissue engineering, but none have so far shown any significant clinical outcome. This is because the pathophysiology, micro biomechanics and molecular mechanisms of disc degeneration are poorly understood. (5) As the ‘building blocks’ of disc tissue, collagens play a vital role in the functional integrity of discs. It has been proposed4 that the collagen’s structural disturbance and cross-linking destruction contribute significantly towards DDD. But there is no data on collagen levels comparing normal and abnormal mechanical properties. Since the problem is at molecular level, we plan to conduct mechanical testing at micro and molecular levels. (6)Long term impact: This study will quantify the mechanical properties of human disc collagens from both normal discs and discs at different grades of degeneration. The new information should provide guidelines for developing and evaluating new tissue engineering techniques to treat and reconstruct degenerated discs. DDD is related to the mechanical functionality of discs, which research has yet to fully resolve. (7)Hopefully, our study will contribute significantly towards understanding the mechanical properties of disc collagens and their influence on the initiation and progression of DDD. This knowledge will influence future research and eventually contribute towards new treatment strategies such as gene therapy and tissue engineered intervertebral disc replacements. Ultimately, our study will benefit all patients suffering from low back pain, and reduce the cost to society as a whole through reduced compensation payments, sick leave, lost workdays and hospital treatment costs.

Abstract:

(1) Optimize and commercialize three new brands of Sr-HA bone cement to fulfill the application in three major areas: (i) Osteoporotic spinal fracture (Sr-HA Spine), (ii) Total hip replacement implant augmentation (Sr-HA Hip) and (iii) Bone tumor, or bone cavity filler (Sr-HA Filler). (2) Provide a complete mechancial and biocompatibility profile of the modified Sr-HA cement formulation as per the ISO 5883 and ISO 10993 standards. (3) Carry out 30-35 cases of clinical trials on bioactive bone cement as a total hip replacement fixation agent. (4) Filing the standards for application to local or international medical device licensing organizations for marketing. The key issue of this study is to optimize present Sr-HA bioactive bone cement by modifying its composition to fulfill a wied variety of orthopaedic application requirements while preserving its desired properties of bioactivity and osteointegration. Ultimately, we aim at replacing the current non-bioactive PMMA bone cement with bioactive bone cements suitable for the particular needs which can significantly reduce clincial complications and improve the quality of life among the patients.

Abstract:

Intervertebral disc degeneration is frequently cited as a cause of low back pain as it would lead to changes in mechanical properties of the disc, causing instability of the spine. In most of the cases, “black disc” and “high intensity zone” in MRI are diagnostic signs of disc degeneration but there is no definite evidence that this is the gold standard for diagnosis of low back pain. With the development of the MRI techniques, Magnetic Resonance Elastography (MRE), it allows in-vivo measurement of the mechanical properties of different tissues. MRE is a phase-contrast-based technique that can directly visualize and quantitatively measure local values of shear modulus, elasticity and stiffness by propagating acoustic strain waves in tissue subjected to harmonic mechanical excitation[1, 2]. Since the discovery of MRE, studies have been performed in animal tissue[3, 4], human brain[5] and human skeletal muscle[6] to find the mechanical properties of different tissues. MRE has also been shown to be an effective diagnostic tool for breast cancer[7, 8]. However, this technique has not been developed in the intervertebral disc and its application on diagnosis of disc diseases has not yet been explored either. In the current study, we will apply MRE to disc. Objective This study will apply MRE to detect the mechanical property of spinal disc in normal subjects and patients with degenerative disc disease. Abstract:

The objective of this project is to apply perfusion imaging (PI), a magnetic resonance dynamic imaging technique, to investigate the haemoglobin status of the cervical spinal cord, which may reveal the mechanism of surgical treatment on cervical spondylotic myelopathy (CSM). CSM is the most common spinal cord disorder affecting the elderly, and its incidence has increased significantly in recent years with the aging trend in the population in Hong Kong. Currently, the problem faced is that the mechanism of surgical treatment on CSM is not fully understood. The lack of understanding of the pathophysiology of CSM limits the rationale for improved treatment strategies. As well, the limit pathophysiology understanding also creates difficulties in the accurate and precise diagnosis of neurological functional deficits. The key issue here is to obtain a detailed understanding of the compression-associated pathomechanism of CSM and the changes after different surgical treatments. In this study, perfusion imaging will be applied to investigate the haemoglobin status of the cervical spinal cord and the relevant changes regarding different surgical treatments.

Abstract:

Objective: To develop an automatic locking mechanism based on the super-elastic and memory effects of nickel-titanium (NiTi) alloy. This locking mechanism will reduce the conventional post-operation complications induced by implant loosening. Introduction: All current internal and external orthopaedic implant locking mechanisms involve the tightening of a nut against the rod and screw head to form a coupling. Surgeons are required to apply a torque over the couplings when they lock these conventional implants. However, there are no standard guidelines regarding how much torque one should apply to be able to secure the lock. In most cases, the surgeon locks it as tightly as possible, which often fails to secure the implant in the long term. If the torque is too small, this will affect the overall mechanical stability of the spinal construct. Loss of correction and implant failure may also occur due to inadequate mechanical stability [1]. Furthermore, daily body movements can wear the rod and the locking mechanism (fretting) [2,3]. Fretting will inevitably generate debris and the coupling subsequently becomes loose, thereby resulting in some complications such as implant-induced osteolysis, pseudarthrosis, subacute low-grade implant infection, late operative site pain and abnormal metal concentration in serum [2-5]. Therefore, the invention of a new locking mechanism is needed. Our thought is to develop a novel locking mechanism with a self-retightening ability once fretting occurs. Hopefully, we can achieve this idea through the unique properties found in nickel-titanium (NiTi) alloys - the super-elastic and shape-memory effects. The NiTi material is malleable under cold temperatures, and is able to remember and recover its original shape when heated, hence the term "shape-memory" [7]. Super-elasticity is a state when the force is always constant under deformation strains that range from 1% to 8% [6,8]. The way NiTi works is analogous to how elastic materials such as rubber work, where the recovery force is proportional to the degree of deformation. Our previously patented thermal treatment protocol and paper have shown the ability to optimize the super-elastic and memory effect at human body temperature [9,10]. With the use of these special properties of NiTi alloy, the new locking mechanism may have the ability to automatically retighten once fretting occurs. Therefore, the complications induced by implant loosening can be solved. Hypothesis: A new locking mechanism made out of NiTi alloy which has the unqiue properties of super-elasticity and memory effect has the ability of self-retightening once fretting occurs. Therefore, this new locking mechanism is able to tackle the post-operation complications induced by implant loosening. Abstract:

Hypoxia-inducible factors (HIFs) function as a master regulator of oxygen homeostasis in metazoan species. HIFs mediate changes in gene transcription in response to changes in cellular oxygenation. Many physiological, pathological, and developmental processes are regulated by the HIF-related pathways. Accumulating evidences suggest that the HIF pathway plays critical roles in a number musculoskeletal differention and diseases but little is known about the role of this pathway in regulating properties and functions of intervertebral disc cells. Degenerative disorders of the intervertebral disc are common in elderly people wherein the disc cells decline in both number and activity. This proposal is therefore desigened to test our hypothesis that the HIFs-mediated mechanism plays a central role in regulating the survival and functions of intervertebral disc cells under extremely hypoxic conditions in nature, and the manipulation of this pathway will facilitate mesenchymal stem cells to differentiate into disc cell lineages. Accordingly, the particular aims of the proposal are: 1) to characterize the growth properties of disc cell populations isolated from rat and humans and cultured in vitro under normoxia and hypoxia; 2) to investigate the expression pattern of the hypoxia/VHL/HIF axis-related molecules in healthy and degenerative intervertebral discs from newborn, young, adult and aged individuals; 3) to analyze the roles of HIF-1a- and HIF-2a in cultured disc cells by RNAi-knockdown and overexpression techniques; 4) to explore the therapeutic potentials of the HIF pathway in directing the differentiation of MSCs towards disc cell lineages.
