| To determine if plasma adiponectin concentration is reduced, at the time of the oral glucose tolerance test (OGTT), in women who develop GDM in the latter half of pregnancy versus women with normal OGTT; to determine, following the diagnosis of GDM, whether the subsequent changes in adiponectin concentration is associated with obstetric complications and perinatal morbidity; to correlate circulatory concentration of adiponectin with polymorphisms and mutations in the apM1 gene in high risk women with and without GDM, and with the expression of adiponectin in subcutaneous and omental fat cells obtained from biopsy at the time of caesarean delivery for obstetric indications; to correlate antenatal adiponectin concentration and result of the genetic study with the postnatal glucose tolerance status in the women who have developed GDM. |

| To determine and compare the serial changes in maternal blood level of TNF-α and its receptors, C-reactive protein (CRP) , and ferritin in relation to the development of gestational diabetes mellitus (GDM) in Chinese pregnant women with singleton and twin pregnancies; to determine the relationship between changes in maternal blood level of TNF-α and its receptors, CRP and ferritin with the occurrence of complicantions in Chinese pregnant women with and with and without GDM, and whether there is any difference between singleton and twin pregnancies. To correlate the levels of TNF-α and its receptors between maternal blood with cord blood, and between these two compartments with placental size and the placental expression of TNF-α and its receptors. To correlate maternal and cord blood, and placental TNF-α and its receptors with the infant birthweight and development of perinatal complications, and the determine if there is any difference between singleton and twin pregnancies. |

| IntroductionCervical carcinoma is the most common cancer of the female genital tract and is still a major cause of morbidity and mortality in women of Hong Kong. Human papillomaviruses (HPVs) have been found in nearly all cases of cervical cancer, and it has been well established to be an important aetiological factor for the development of the cancer (zur Hausen, 2002). Although HPV infection is generally believed to be the initiating factor for cervical cancer development, the development of fully invasive tumour must involve other factors, such as genetic alterations in oncogenes and tumour suppressor genes, epigenetic alterations in cancer-related genes and changes in gene expression profile, to promote tumour formations.As tumour grows, because of hypovascularization, many cancer cells are often exposed to adverse conditions such as glucose deprivation, insufficient supply of oxygen and other nutrients (Brown and Giaccia, 1998; Dang and Semenza, 1999; Hockel et al, 1999; Semenza, 2003; Giaccia et al, 2004). In this situation, the tumour may become quiescent. However, aggressive tumour may acquire an adaptation to adverse conditions to further growth and development. This adaptative mechanism may involve the up-regulation and/or down-regulation of many stress-responsive genes.Briefly, from others’ work, a number of genes have been shown to respond to stress stimulation and made the cancer cells more resistant to adverse conditions. For examples, pancreatic cell lines but not liver cell lines have been shown to tolerate to nutrient deprivation (Izuishi et al, 2000). Increased expression of hypoxia-inducible factor-1 alpha, and anaerobic metabolism-associated genes including glucose transporter-1 and aldoase A, may contribute to the survival and proliferation of pancreatic cancer cells in hypoxia and glucose deprivation (Akakura et al, 2001). Kato et al. (2002) has demonstrated that elevated expression of AMP-activated protein kinase (AMPK) may directly contribute to tolerance to nutrient deprivation in cancer cells. AMPK antisense RNA expression in pancreatic cancer cells inhibited tumour growth in nude mice. AMPK has been proposed to be potentially useful as a new target for cancer therapy to suppress tumour growth and invasion (Kato et al, 2002). Using comparative genome hybridization, our own work has also demonstrated the occurrence of gene amplification and over-expression of the AMPK alpha 1 subunit in cervical cancer (Huang et al., in press, Huang et al., submitted). It is anticipated that much greater number of stress-responsive genes are awaiting to be discovered using the genome-wide analytical scanning such as cDNA microarray.Objectives:(1) To determine, by in vitro cell culture, the degree of tolerance of cervical cancer cells to stress(2) To identify, using microarray technology, differentially expressed genes in cervical cancer cells under physiological stresses such as hypoxia or nutrient deprivation |

| Lymphedema is a debilitating condition that occurs in up to 40% of patients after pelvic lymphadenectomy. New surgical techniques have been advocated to reduce this complication. A pilot study using Omentoplasty observed that 8% of patients after this procedure had clinical lymphedema compared with 40% in historical controls. This study aims to see whether these preliminary results are validated in a prospective randomised comparison. |
